The chief executive of the Clinton Foundation is admitting the charity made errors with disclosing donor information.

Maura Pally said the foundation plans to refile some of its tax forms because it had "mistakenly combined" government grants with other donations.

"We made mistakes, as many organizations of our size do, but we are acting quickly to remedy them," Pally wrote on the foundation website. "And [we] have taken steps to ensure they don't happen in the future."

The announcement comes as Hillary Clinton's presidential campaign is coming under scrutiny for donations to her family's foundation that came when she was secretary of state.

The New York Times has reported Russian donors gave millions to the Clintons while the State Department approved the sale of U.S. uranium mines to Russia.

The extensive Times investigation was launched based on a tip from the new book Clinton Cash by Peter Schweizer.

On ABC's "This Week," Schweizer said, "What's important to note is it was confirmed on Thursday, both by The New York Times and The Wall Street Journal, that there were multi-million dollar, non-disclosed donations that were made to the Clinton Foundation that were never disclosed by the Clintons. This is a direct breach of an agreement they signed with the White House."

Hillary Clinton has dismissed the reports as conspiracy theories. But Schweizer says he sees evidence of a trend in controversial donations.

"When you ever have an issue of the flow of funds to political candidates, whether that's to their campaigns, whether that's to private foundations, whether that's to their spouse, is there evidence of a pattern of favorable decisions being made for those individuals? And I think the point that we make in the book is that there is a troubling pattern. There are dozens of examples of that occurring," Schweizer said.
